What We Do

Home Based Primary Care (HBPC) is an interdisciplinary primary home care program that provides intermittent care to Veterans with Chronic longitudinal healthcare needs.

Service Information

The entire team is involved with ALL Veterans who are admitted to the program.  Visits are made to the Veterans in their homes for the purpose of enhancement of quality of life.  The team consists of Mid-level Providers, RNs, Social Workers, Occupational Therapist, Psychologists and Dietitians.  The Veteran becomes a part of the team and is engaged in the development of a plan of care that assists them in attaining an improved quality of life.  Calls are directed to the main office. Caregivers are supported with options for self-care and resources available within the individual communities.  This program provides coverage in all areas of the Southwest and West Michigan area.
